Add the following to the card.php file found here:
../wp-content/plugins/slim

vcard(); ?>

To disable search and the category drop down can be done by simply setting a couple shortcode options that Slim Plus enables. Like so:

[connections enable_category_select='FALSE' enable_search='FALSE']
