Question … you want the form only to show for logged in users so they can add their own entry to your directory. Is this correct?
Navigate to the Connections : Settings admin page and click the Display tab. Scroll to the Result list actions section and enable the enable the “View My Directory Entry” action link and “Edit my Directory Entry” entry action option. Leave the “Submit New Entry” option disabled.
Next, navigate to the Connections : Roles admin page and make sure the user roles you want to be able to add/edit their entry have been assigned the correct capabilities as explained on this doc page:
