I would suggest commenting out the field arrays that you wish to remove. I’ve never really fully tested changing the 'show' => TRUE, field option. That more or less exists for future development.

RE: Adding the option of which image to display

Ok, I’ll plan on adding this option next week and releasing it. I’ll also try to squeeze in an update to Form so it respections the Field Configuration settings for the name field. In theory, you could probably not need the code snippet any longer. That’s something you could test after both updates are released — to ensure your requirements are still met.
