The easiest way to add Form is by adding its action link above the directory. Here’s the link to the documentation on how to enable the Form submit action link.
Now, since you are using Link, I am going to assume you actually only wish to allow your registered users to add and maintain their entry. If that is the case, then the instructions you should follow are those for configuring Link as recommended in the link I gave for adding the Form submit action link.
As far as editing fields, there is no way to edit fields as Form is to be more or less plug and play. You can turn off and arrange fieldsets on the Form settings admin page.
Most of the time when someone says they want to edit a field, they generally are asking how they can change the label of the field. If that is what you are asking, here’s the link that explains how to change text within Connections.
There is a shortcode for Form that you can use to display Form on any page. It is [connections_form]. However it is not recommended to use it because all the internal links that are created automatically/internally by Connections will be to it’s page and not the one you you add the shortcode on. This includes Link.
