In the widget configuration panel, you can choose not to display the date. Under the Show Image drop down, select None.

Also, to fix the display, edit the page, switch the editor to the Text tab and remove the code and/or pre HTML tags from around/in the widget shortcode.

To get the columnar layout, you need to apply some custom CSS so the list is displayed in columns instead of a single column.
