The correct text domain for Connections is connections not Connections. I know that is not what you are asking but I wanted to point it out.

The “Notes” header in Excerpt Plus can not presently be changed using Say What. The next update to the template will address that as well as add support for the Template Customizer.

You need to use the str_note_head shortcode option. Here’s a link to FAQ which explains how to use it.. All the string shortcode options For Excerpt Plus are here.
