I’m sorry, let me clarify.

The way I think this is done is that your example is one form. Each dropdown selects different child categories, a text field for the Zip, dropdown for the radius and a text field for the Search. Then, you send the variables on submit (Search) to a “do shortcode” php script, using the variables from the form to output the results via the built in short code capabilities. So, the short code output is set by the form, the categories, the zip code radius, etc.

If that is not how it is done, would the way I lay out be possible?
