Probably the better solution would have been to configure cMap to use the image rather than the logo:

[connections image='photo' tray_image='none' str_image='No Photo Available' str_tray_image='No Logo Available']
I see you’re using an older version of Form. You should probably update it to the latest version. It is available to you in your purchase history.

After you update, the refer to this QuickTip on how to change text within Connections.

The original text you’ll want to enter is: Select Logo.

Oh, after you update, make sure to visit the Form settings on the Connections : Settings admin page.
