That is because you must have switch back to the Default Card template. Switching it back to the cMap template on the Connections : Templates admin page will bring back those features which are part of that template.

Also, the screenshot of the shortcode, you do not need the enable_search, enable_category_select, enable_map, enable_note or page_limit options because those are the default values. you only need to add them if you want to turn them off or change the page limit.
