Yes, I suggest cMap or any of the *Plus templates. The Widget Pack is an option as it outputs a list of clickable links of the categories that can be placed in the theme’s sidebar.

The Widget Pack is the simplest solution. The Market template has a built in category list but is not responsive theme friendly.

Sorry, not sure if I understand. My best guess is you mean the notes field in the admin when adding an entry, yes? If it is not displaying, click the “Screen Options” tab in the upper right and make sure it is enabled. If it is enabled, you might have collapsed it. If it is collapsed it is still there but will only show as a thin bar with the Notes title.
