The best answer it to refer you to a few free extensions I’ve release for Connections … all showing, in code, how to add custom fields. Please take a look at these extensions:

Say What? is the method I’ve been recommending on changing fields labels … sorry I really can not recall what I may have meant by renaming fields and in what context.
