Sorry, impossible to say without seeing the code that you’ve created to add the custom fields. The only thing I can say is the custom field API doesn’t really care if it is an existing entry or a new entry other than checking to see if it should add or update existing custom fields. And the primary difference is the database queries being made.

Throw you code up on Github, I’ll take a quick look to see if anything jumps out at me.
