Sorry, no there is no point and click way of adding additional form fields. Adding a phone fields would require code changes.

The email subject and email message body are for the sender to fill in. It is not meant to have pre filled content. These are also required fields, which means the fields will have to be filled by the user in order for the message to be sent.

I do not have any plans at this time to add captcha support as it highly discourages user engagement. Contact does have other anti spam features built in which do not negatively affect user engagement.
