I would like to apologize for my delay in my reply as I was not receiving my notices for new support forum posts for about a week. Just reply notices to ones that were already started which is why I’m just now noticing.

Yes, Form is still available. Somehow, and I can’t imagine how, the page was trashed. I’ve restored the page.

I checked the order status. It says pending because PayPal hasn’t recorded the transaction as completed. I’ve checked, no funds have been transferred. there have been many sales since you attempted your purchase so it is working. Not sure what the issue may have been.
