I just answered this in your other forum post. Multiple posts aren’t necessary … it actually just makes it take longer for me to reply because these more to jump back and forth to reference to make sure I’m answering your question accurately and completely.

I’ve looked at the shortcode you’re using, just a few typos, I’ve corrected them below. Since it is long, you’ll have to triple click on it it, then you can copy the whole thing.

[connections_form default_type='organization' select_type='false' str_contact_name='Business Contact Information' category='true' photo='true' logo='true' address='TRUE' phone='TRUE' email='TRUE'  link='TRUE'  notes='TRUE' bio='false' social='false' messenger='FALSE']

Other than a few small typos, it looks like your shortcode is spot on for the info you want to capture.

I just took a look at your site. It looks like you have it mostly there. Correcting the shortcode should got it to default to an Organization and change “Entry Name” to “Business Contact Information”.
